Adult Social Care encompasses a wide range of vital services covering;
- Commissioning Team â€“ we commission and work with a number of providers, to ensure that people with care needs have access to a range of good quality support to enable them to live independent and fulfilling lives
- Social Work Services – we work with individuals who have care and support needs, and with their carers be they older or younger adults
- Occupational Therapy and Equipment Services – we aim to enhance peopleâ€™s ability to live safely and independently in their home
- Internally Provided Services â€“ we provide direct support to people enabling those in most need to live independent and fulfilled lives
- Business Systems and Continuous Improvement – we provide a range of services including Financial Assessments, provider payment processing and Business Systems administration. The Business Systems team provide support to Adult Social Care, Childrenâ€™s Services and Education Services.

What is the job role?
The successful applicant will be required to work in partnership with older adults with care and support needs, carers, health and other social care professionals, providing assessment and care management services.
The social work post will be based within Community 65+ services which covers intake, case management, reviews and a Continuing Health Care (CHC) team. You will be required to comply with relevant legislation, including the Care Act, the Mental Capacity Act, Mental Health Act. Data Protection Act (2018 (GDPR principles).
You will be expected to promote strength based social work, and support people to contribute to their community. This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).
Who are we looking for?
You will be a Social Worker registered with Social Work England and will have a relevant qualification, such as a Degree in Social work, Dip SW,CSS, CQSW or validated equivalent in another country.
We offer the Assessed and Supported Year in Employment (ASYE) programme for newly qualified Social Worker’s.
You will have knowledge of, appropriate legal statutes including the Care Act 2014,
Mental Capacity Act 2005 and Deprivation of Liberty Safeguards, Mental Health Act 1983, and a knowledge of the Children and Families Act 2014.
