About the role 

Are you a qualified Social Worker seeking a rewarding hospital-based role with scope for autonomy and professional growth? Join our South Transfer of Care Team, covering Addenbrooke’s Hospital, other local surrounding hospitals and Community Rehabilitation Units.

 You will work with adults aged 18+ to support safe discharges, arrange care and support, and undertake the six-week post-discharge review to ensure ongoing needs are met and outcomes are sustained.

Interviews will likely be held during the week commencing 21st September.

About you 

We are looking for confident and dynamic Social Workers who are passionate about making a difference in their local community.

You will be a qualified Social Worker with current Social Work England registration and be able to travel independently across the county and work from multiple locations.

You will have experience of working with adults, an understanding of relevant legislation including the Care Act 2014 and Mental Capacity Act 2005, and be comfortable managing a busy caseload while maintaining a focus on safety, independence and positive outcomes.

You will be an effective communicator, able to build positive relationships with individuals, families, carers and partner agencies, and thrive in a collaborative multidisciplinary environment
