We are seeking to appoint a Social Care Worker with experience of adult social care.
The East Older Person Social Work Team cover the Middlesbrough area. The service is very keen to promote flexible working for all staff. This includes agile working at home as well as Woodside Community Centre.

The role will involve:
Assessing individuals both service users and carers who live in Middlesbrough
Completing comprehensive care needs assessments and support plans
Implementing services for those who are vulnerable and are experiencing mental health issues.

Salary: £32,654 – £34,314
