About the role

An exciting opportunity has arisen for A Front of House Social Worker to be based at  Addenbrookes Hospital.

The role requires an experienced worker with a good knowledge of social and health pathways of support. Working autonomously alongside therapy and health colleagues within the Accident and Emergency Department of the hospital, you will ensure prevention support with timely discharge avoiding the need for admission wherever possible.

You will work alongside The Adult Early Help team who provide advice and support to anyone over the age of 18, who is seeking support or social care and will be part of our Prevention and Early Intervention services made up of Social Workers, Occupational Therapists, Adult Support Coordinators and Welfare Benefit Advisors from a variety of backgrounds. You will be part of the service offering support to individuals and also to families and carers.

As a council Cambridgeshire will support individuals to look at their individual learning outcomes and offer a wide range of training and apprenticeships.

IMPORTANT: Roles based in Cambridge University Hospital will require you to able to evidence that you have had a number of vaccinations, or to be willing to undergo a blood test to evidence this. If you have not had the required vaccinations, you may be required to do so prior to starting employment. As part of your pre-employment checks you will need to provide evidence of your vaccinations. Your employment will not commence until this evidence is received. About you

You will have a good understanding of all relevant legislation to a level where you can confidently challenge health colleagues where required. You will also have experience of assessing needs and providing support. A good knowledge of the health system and the support it offers is important.

You will need to have good and accurate literacy, numeracy and administrative skills and the ability to analyse information to solve general queries.

It is imperative that you have strong communication skills, a confident telephone manner and can work on your own initiative.

Good planning and organisation skills will be key, and you should be used to working under pressure to meet deadlines, allowing flexibility and resilience.

You will need a good working knowledge of the Care Act 2014 and Mental Capacity Act 2005. Experience of multi-disciplinary working to enable best outcomes for individuals.

You must be a Qualified Social Worker with current Social Work England Registration
