We are currently seeking a part-time Face to face Health Coach (0.5FTE) for our locations in Devon covering the areas of Plymouth, Totnes and Okehampton.

The Role:
As an Oviva Health Coach, you will collaborate with our diabetes clinical team to deliver our innovative diabetes remission services to patients both face-to-face and remotely (video, phone, app). Your primary responsibility will be providing high-quality, evidence-based clinical care tailored to individual patients' needs. You will work alongside a dedicated clinical delivery team, including Clinical Team Managers, Health Coaches, Dietitians, and Patient Support Services, to ensure exceptional patient-centric care and meet service referral requirements.

Key Responsibilities:

  • Engage in one-on-one or group patient consultations through remote phone or video calls, as well as in-person meetings, utilizing our company app for coaching sessions.

  • Apply your expert knowledge of diet and nutrition to support patients with evidence-based dietary approaches.

  • Utilise behavioral change and motivational techniques to guide and support patients throughout their program journey.

  • Provide support to medically and psychologically complex patients, including those with multiple comorbidities.

  • Maintain accurate and up-to-date clinical records using our secure electronic medical record system, ensuring the proper management of sensitive data.

  • Complete relevant training courses and stay updated with changes in program processes.

  • Safely manage clinical risk incidents, adverse events, and out-of-range readings according to established protocols.

  • Adhere to the Safeguarding policy and protocol and the Incident policy at all times.

  • Demonstrate autonomy in clinical practice and responsibly manage clinical risks, seeking professional guidance when necessary.

    What are we looking for you to bring :

  • Completion of the Cambridge Diabetes Education Programme or SCOPE obesity training, or a CPD plan to complete these, would be a strong advantage.

  • Experience in delivering a Diabetes Remission program and managing safeguarding scenarios would be highly desirable.

  • Proficient in using data and software, including electronic patient record systems.

  • Emotional resilience and the ability to thrive in challenging situations.

  • Evidence of ongoing Continuing Professional Development (CPD) post-registration and compliance with AfN registration requirements.

  • Proven experience managing a large caseload independently and within a team.

  • 2:1 degree in an accredited Nutrition course (RfN with AfN membership) or an appropriate degree in Dietetics, along with current dietetic registration with HCPC.

  • Experience in the nutritional management of patients with complex health conditions.
